Community Health Worker

LMAS District Health Department is accepting applications for a Community Health Worker (CHW) to serve in Luce and Mackinac counties. The CHW will facilitate needed health and human services for pregnant mothers/infants with history of, current or at risk for, substance use disorders to secure better health outcomes. CHW’s are valuable to their clients by being culturally and geographically connected to the communities they live and work in. In partnership with the Maternal Infant Health Program (MIHP) and Healthy Families UP, the CHW links clients to community resources, and assists them in overcoming barriers to health, social services, education and employment. Possession of a high school diploma or equivalent is required, CHW credentials and experience with social services, substance use disorders and public health is preferred. Possession of a valid MI driver’s license, reliable transportation, and ability to travel is required. Excellent customer service skills, knowledge of maternal/infant health, and knowledge of core functions and essential services or public health is a plus.

FAMILY SUPPORT WORKER

LMAS District Health Department is accepting applications for a Family Support Worker based in our St. Ignace office, to provide services in Chippewa and eastern Mackinac counties. This position is full-time with benefits. Job duties include but are not limited to: initiating and maintaining regular and long-term support with families, primarily in the family’s home, and, in particular, with parents who are working with MDHHS Child Protective Services. The candidate will assist parents in improving their skills to optimize the home environment; improving the family support system; increasing the family’s ability to problem solve and assume the role of advocate for themselves and their children. An Associate’s or Bachelor’s degree in a human services field is preferred, a valid driver’s license is required. Must be available to families after-hours on an emergency basis. Work experience with children 0-5 years of age with a knowledge of child growth/development and parent-child relationships preferred. Travel will be necessary.
