IRREGULAR PART-TIME POSITION

LICENSED SOCIAL WORKER AND/OR REGISTERED NURSE

. Provide home visiting for pregnant women and infants in

Delta and/or Menominee County.

. Possession of a MI driver’s license, reliable transportation,

and ability to travel.

. Excellent customer services skills and a min. of 1 year of

maternal/infant care experience a plus.

. Must hold a current Michigan license in social work or nursing.

Through home visiting, provide case management, care coordination, support services,

and education to families. Function independently as part of a multi-disciplinary team.
