1/2 TIME POSITION WITH THE POSSIBILITY OF FUTURE FULL TIME POSITION

LICENSED SOCIAL WORKER AND/OR REGISTERED NURSE

Provide home visiting for pregnant women and infants in Delta and/or Menominee County.
Possession of a MI driver’s license, reliable transportation, and ability to travel.
Excellent customer services skills and a min. of 1 year of maternal/infant care experience a plus.
Must hold a current Michigan license in social work or nursing.

Through home visiting, provide case management, care coordination, support services, and education to families. Function independently as part of a multi-disciplinary team.
